Is there any way to salvage this, short of removing and replacing the veneer itself? Grain is split and raised, presumably from moisture damage.

You can try a clothes iron, with/without steam, and some serious pressure. Experiment with the temps. The glue is still there, so it may reactivate.

Sure! All you can do is try. It may not make it better, but I don't think you'll do any more damage. Be sure to use a cotton cloth under the iron.
