No I only tightened in the one position. I'll tighten in all three next week when the new bits arrive.

My 120v. Makita hammerdrill has a keyless chuck and I don't recall ever having a problem with it. I think most of my percussion bits are smooth shanked, though, one or two of them have a flat on one side.

Yep. Normally a requirement to keeping keyed chucks tight on bits, especially with a worn chuck.

Grind a flat or 2 on your existing bits where they go in the chuck. Tighten all 3 holes.
BTDT...worked fine.

Tapcon bits are designed for hammer drills. Its not just the part that goes into the chuck but the tip and design of the whole bit is different too. Cheap masonary bits are not designed for hammer drills, the package will probably say something like "rotary only", or "not for hammer drills".
